Please see an example here of the filled out form. Please note that all fields below do not have to be filled out although at minimum we need at least 1 URL and 1 Keyword/Anchor text for each article, if you choose only 1 URL and 1 Anchor per article you will receive only that when we write the article.. Keep that in mind if you do not wish to fill in all of the fields. If you have additional requirements please add them in the "Comments or additional Instructions" box.
